چکیده

Background: Individuals at ultra-high risk (UHR) for psychosis are understood to be the putative prodrome of overt psychosis, although there is still a possibility of false positives. The aim of the present study is to examine the potential links between multi-dimensional schizotypy, basic symptoms and emerging psychosis as defined by positive symptoms in individuals at ultra-high risk (UHR) for psychosis. Method: Sixty-one UHR individuals and 41 healthy controls were participated in baseline assessments. For multi-dimensional schizotypy assessments, Chapman’s perceptual aberration scale, magical ideation scale, revised physical and social anhedonia scales, schizotypal ambivalence scales, and Eysenck’s impulsiveness scale were used. Basic symptoms were assessed by using the schizophrenia-specific items of the Frankfurt complaints questionnaire (FCQ). Results: Individuals at UHR showed higher schizotypy scores and basic symptoms at baseline. The transition rate of overt psychosis was 36.4 % at 4 years of follow-up. Cox regression analysis showed that the basic symptoms (HR 1.456, p = 0.025) and impulsiveness (HR 1.175, p = 0.039) were significant predictors of transition into overt psychosis in UHR individuals [χ2(4) =14.242, p = 0.007]. Conclusion: The addition of the self-reported basic symptoms and impulsiveness may be useful for a risk enhancement or stratification strategy in individuals at ultra-high risk for psychosis. Pre-morbid Conduct Disorder symptoms are associated with cannabis use among individuals with a first episode of psychosis.

BACKGROUND Early cannabis use has consistently been associated with an increased risk for the later development of psychosis. Studies suggest that Conduct Disorder (CD) is more common amongst young people who later go on to develop psychosis. CD has been associated with greater and earlier cannabis use in general population samples. Based on this evidence, we hypothesised that among patients ex...

Cannabis use and first-episode psychosis: relationship with manic and psychotic symptoms, and with age at presentation.

BACKGROUND Cannabis use has been reported to be associated with an earlier onset of symptoms in patients with first-episode psychosis, and a worse outcome in those who continue to take cannabis. In general, studies have concentrated on symptoms of psychosis rather than mania. In this study, using a longitudinal design in a large naturalistic cohort of patients with first-episode psychosis, we i...

Poor medication adherence and risk of relapse associated with continued cannabis use in patients with first-episode psychosis: a prospective analysis

BACKGROUND Cannabis use following the onset of first-episode psychosis has been linked to both increased risk of relapse and non-adherence with antipsychotic medication. Whether poor outcome associated with cannabis use is mediated through an adverse effect of cannabis on medication adherence is unclear. METHODS In a prospective analysis of data acquired from four different adult inpatient an...

Gyral and sulcal cortical thinning in adolescents with first episode early-onset psychosis.

BACKGROUND Psychosis is associated with volumetric decreases of cortical structures. Whether these volumetric decreases imply abnormalities in cortical thickness, surface, or cortical folding is not clear. Due to differences in cytoarchitecture, cortical gyri and sulci might be differentially affected by psychosis. Therefore, we examined differences in gyral and sulcal cortical thickness, surfa...

The Effect of Cannabis Use and Cognitive Reserve on Age at Onset and Psychosis Outcomes in First-Episode Schizophrenia

OBJECTIVE Cannabis use is associated with a younger age at onset of psychosis, an indicator of poor prognosis, but better cognitive function, a positive prognostic indicator. We aimed to clarify the role of age at onset and cognition on outcomes in cannabis users with first-episode schizophrenia as well as the effect of cannabis dose and cessation of use. METHODS Ninety-nine patients without ...
